Job summary

Qualifications

• Current, active Maryland RN license.
• Experience in a skilled nursing or long-term care setting preferred.
• MDS experience strongly preferred.
• Knowledge of the MDS/RAI process and long-term care regulatory requirements.
• Strong clinical assessment and documentation skills.
• Ability to organize and prioritize multiple assessments, deadlines, and responsibilities.
• Strong communication and interpersonal skills.
• Ability to work effectively with an interdisciplinary team.
• Proficiency with electronic medical records and standard computer applications.

Responsibilities

• Coordinate and complete MDS assessments according to required schedules and regulatory guidelines.
• Review resident medical records and other documentation to support accurate MDS completion.
• Conduct resident interviews and gather information from residents, families, staff, and others as needed.
• Collaborate with interdisciplinary team to obtain accurate information for MDS assessments.
• Identify changes in resident condition that may require additional assessment or care plan intervention.
• Participate in and/or coordinate interdisciplinary care plan meetings.
• Develop, update, and maintain resident care plans based on assessments.
• Monitor MDS schedules, deadlines, and required documentation for timely completion.
• Maintain knowledge of CMS, federal, state, and facility requirements related to MDS and documentation.
