Job summary

Liberty Health in Fayetteville, NC seeks an experienced MDS Coordinator (RN license required) to lead the MDS process and care planning across disciplines. You will ensure timely MDS completion, data accuracy, and compliant submissions, coordinating RCP meetings and communicating with staff and supervisors.

Ideal candidates have RN licensure, familiarity with MDS-RAP, Care Planning, and preferably RUG-IV and MDS 3.0 experience; you must follow facility policies and state/federal guidelines.

Qualifications

• RN license required (North Carolina Board of Nursing) and ability to follow facility policies and procedures.
• Experience with MDS-RAP and Care Planning functions.
• Prefer experience with RUG-IV.
• Prefer experience with MDS 3.0.

Responsibilities

• Maintains and follows a schedule of due dates for all MDS.
• Coordinates the completion of the MDS by all disciplines.
• Reviews the MDS data for accuracy and meets with appropriate staff.
• Completes the MDS and inputs into the computer.
• Verifies that assessments have been transmitted and approved by the State in a timely manner.
• Corrects any rejected records and prepares them for re-submission.
• Completes CAAs according to state guidelines.
• Formulates Resident Care Plan (RCP) with interdisciplinary team.
• Schedules and conducts RCP meetings regularly.
• Communicates the RCP to staff and notifies supervisor of noncompliance.
• Works with all departments to ensure understanding and compliance with the RCP.
• Performs other related duties as directed by DON/Administrator.
