Job summary

American Baptist Homes of the Midwest – Thorne Crest Health Center in Albert Lea, MN is seeking an MDS Coordinator-Registered Nurse to join the team. This part-time day-shift role focuses on coordinating assessments and care planning to uphold quality resident care.

You’ll transmit MDS assessments, ensure compliance with federal and state regulations, and collaborate with the interdisciplinary team to support timely, accurate resident evaluations.

Qualifications

• Active Minnesota RN license.
• At least 1 year of nursing experience.
• Strong critical thinking and multitasking in a fast-paced environment.
• Ability to read, write, and speak English.

Responsibilities

• Coordinate and oversee the full collaborative, interdisciplinary assessment and care planning process in our skilled nursing facility.
• Transmit MDS assessments, correct rejected assessments, and adhere to MDS and RAI requirements.
• Ensure all residents are assessed according to established timeframes and in compliance with federal, state, and Medicare regulations.

What You'll Do:

• Coordinate and oversee the full collaborative, interdisciplinary assessment and care planning process in our skilled nursing facility
• Transmit MDS assessments, correct rejected assessments, and adhere to the Minimum Data Set (MDS) and resident assessment instrument (RAI) requirements
• Ensure all residents are assessed according to established timeframes or changes in status and in compliance with federal, state, and Medicare regulations

What You'll Need:

• Active MinnesotaRN License
• 1 year of nursing experience (required)
• Strong critical thinking skills, independent judgment, and the ability to multitask and prioritize work assignments in a fast-paced work environment
• Ability to understand, read, write, and speak English
