Job summary

Northern Pines Rehabilitation and Nursing in Cut Bank, MT, is seeking an experienced MDS Coordinator to join our in-person team. The role involves assessing residents and coordinating care plans across disciplines to meet Medicare requirements and ensure holistic, individualized care.

Qualifications

• Must be able to pass a background check.

Responsibilities

• Assess residents' behavioral, psychological, clinical, nutritional, and physical strengths and needs to create an individualized care plan.
• Collaborate with nursing, PT, nutrition, and other teams to ensure care plan guidelines are followed and compliant with Medicare requirements.

MDS Coordinator Responsibilities:

Assess residents behavioral, psychological, clinical, nutritional, and physical strengths and needs to create an individualized cared plan that considers the resident holistically. The MDS Coordinator also works with nursing, PT, nutrition, etc., to ensure care plan guidelines for each resident are followed, and compliant with ethical standards and Medicare requirements.

MDS Coordinator Requirements:

• RN license preferred
• Experience working in a Skilled Nursing Facility.
• Must be able to pass a background check.
