Job summary

Diversicare of Foley is seeking an experienced MDS Coordinator to manage the RAI process under RN supervision and work with the care team to optimize resident care. The role emphasizes timely MDS assessments, accurate coding, and collaborative care planning to ensure regulatory compliance.

Responsibilities include leading Case Management meetings, maintaining documentation, and supporting quality improvement initiatives in a post-acute setting.

Qualifications

• Must hold a current nursing license in the state of employment.
• Experience in Long Term Care.
• Knowledge of Medicare, Managed care and RAI Process.
• Proficiency with computer systems and EHRs.

Responsibilities

• Facilitates the RAI Process under the direction of an RN or DNS.
• Collaborates with the interdisciplinary team to identify patient/resident needs.
• Coordinates and conducts daily Case Management meetings.
• Ensures timely MDS assessments per state and federal regulations.
• Ensures MDS coding is accurate through chart review and staff interviews.
• Maintains MDS schedules and care plan conferences.
• Reviews state and federal reports and maintains required documentation.
• Supports quality measures and process improvements.
