Diversicare is seeking a dedicated MDS Coordinator to join our nursing team in Foley, AL. You will manage the RAI process, coordinate interdisciplinary care, and ensure accurate MDS coding and timely submissions in compliance with state and federal regulations.

You will collaborate with nurses, HIM staff and center leadership to optimize care plans, quality measures, and regulatory reporting. A current nursing license and LTC experience are required, with strong computer and regulatory knowledge.

Formación

• Must hold current nursing license in the state of employment.
• Experience in Long Term Care.
• Computer skills.
• Knowledge of Medicare, Managed care and RAI Process Required.
• Participating in the LPN to RN Back to School transition program.

Responsabilidades

• Facilitates the RAI Process under the direction of an RN or DNS.
• Collaborates with the interdisciplinary team to identify patient/resident needs.
• Coordinates and conducts the daily Case Management meeting.
• Ensures timely MDS assessments per state and federal regulations.
• Ensures interdisciplinary team completes designated sections of the MDS assessment.
• Ensures accurate coding of the MDS assessment from chart review and interviews.
• Maintains the MDS assessment tracking system (timelines and due dates).
• Facilitates Care Plan conferences with the interdisciplinary team, patient and families.
• Obtains, reviews and maintains all state and federal reports and makes corrections timely.
• Monitors Quality Measures and ensures MDSs support and reflect the Quality Measures.
• Provides education related to the RAI Process.
• Coordinates electronic submission of required documentation to state databases and other entities.
• Ensures Medicare/Medicaid regulatory guidelines are completed accurately and timely.
• Provides Managed Care case management at the center level.
• Reviews ADRs with HIMC to ensure docs are available.
• Participates in billing reviews to ensure claims have supporting MDS documentation.
• Participates in QA and Process Improvement activities.
• Keeps knowledge base updated on Medicare, Managed Care, Medicaid, RAI Process and tech updates.
• May fulfill duties of a direct care nurse as needed.
