Diversicare Foley is seeking a skilled MDS Coordinator to join our team in Foley, AL. The role focuses on facilitating the RAI process under RN/DNS supervision, coordinating daily case management, and ensuring accurate MDS coding and timely submissions in compliance with federal and state regulations.

The ideal candidate will have an RN license in Alabama, experience in long-term care, and strong computer skills to support regulatory reporting and quality measures.

Qualifications

• Must have current nursing license in the state of employment.
• Experience in Long Term Care.
• Computer skills required.
• Knowledge of Medicare, Managed care and RAI Process.

Responsabilités

• Facilitates the RAI Process under the direction of an RN or DNS.
• Collaborate with interdisciplinary team to identify patient/resident needs.
• Coordinate and conduct the daily Case Management meeting.
• Ensure timely MDS assessments per state and federal regulations.
• Ensure interdisciplinary team completes MDS sections.
• Code MDS assessments accurately from records, observations and interviews.
• Maintain MDS scheduling tracking system.
• Facilitate Care Plan conferences with the team, patients and families.
• Obtain and review all required state and federal reports.
