Qualifikationen

• Must hold current nursing license in the state of employment.
• Experience in Long Term Care.
• Computer skills required.
• Knowledge of Medicare, Managed care and RAI Process required.

Aufgaben

• Facilitates the RAI Process under the direction of an R.N. or DNS.
• Collaborates with the interdisciplinary team to identify patient needs.
• Coordinates daily Case Management meetings.
• Ensures timely MDS assessments according to regulations.
• Facilitates Care Plan conferences with the interdisciplinary team.
