Job summary

Gallup Nursing & Rehabilitation is seeking an experienced MDS Coordinator (RN) to ensure timely and accurate completion of the RAI process and care management from admission to discharge, in line with policy, regulations, and guidelines. You will work under the Director of Nursing and VP of Clinical Reimbursement to coordinate information systems and educate the clinical team.

Qualifications

• RN license required.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Understanding of Quality Indicator process, OBRA regulations and MDS.
• Knowledge of the care planning process.
• Experience with MDS 3.0 preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on necessity of MDS continuations based on clinical criteria per the MDS User’s Manual.
• Assist with coordinating daily stand-up meetings and setting assessment reference dates.
• Ensure compliance with federal and state regulations for RAI processes.
• Monitor MDS and care plan documentation to support MDS coding in medical records.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data into MDS programs accurately and consistently.
• Code MDS information from medical records, observations, and interviews.
• Attend team meetings to relay changes and update records.
• Support DON with changes in resident status and verbal reports to reflect updates.
• Continue updating knowledge of data entry and computer technology.
• Submit electronic documentation to state databases and other entities as policy dictates.
• Ensure final MDS completion and data submission to relevant authorities.
• Instruct staff in RAI, PPS Medicare/Medicaid and related clinical systems.
