Job summary

Outfield Healthcare Partners seeks an experienced MDS Coordinator to manage timely RAI and care management from admission to discharge in line with policy and regulatory guidelines.

Under the guidance of the Director of Nursing and VP of Clinical Reimbursement, you will coordinate information systems operations and staff education for the clinical team, ensuring compliant documentation across care transitions.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Thorough understanding of MDS 3.0, PPS, and Medicaid reimbursement.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments.
• Determine necessity of additional MDS assessments based on criteria.
• Coordinate daily stand-up meetings and set assessment reference dates.
• Ensure compliance with federal and state regulations for MDS processes.
• Monitor and document MDS and care plans in the medical record.
