Job summary

Outfield Healthcare Partners is seeking an MDS Coordinator to ensure timely and accurate completion of the RAI and care management process from admission through discharge, in accordance with company policy and Federal, State, and Certification guidelines.

Under direction of the Director of Nursing and VP of Clinical Reimbursement, you will coordinate information systems operations, educate the clinical team, and maintain documentation integrity while ensuring confidentiality and regulatory

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process and OBRA regulations.
• Knowledge of the care planning process.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide if additional MDS assessments are needed based on clinical criteria.
• Coordinate daily stand-up meetings to review resident care and assessment reference dates.
• Comply with federal and state regulations for MDS processes.
• Monitor MDS and care plan documentation in medical records to support coding.
• Maintain current MDS status per guidelines and ensure accurate data entry.
• Code MDS accurately using information from medical records, observation, and interviews.
• Attend interdisciplinary team meetings to communicate changes and update records.
• Assist DON with significant changes, physician orders, and changes reflected in MDS and care plans.
