Job summary

Rolling Hills Healthcare is seeking an MDS Coordinator to lead accurate MDS assessments and ensure regulatory compliance in a skilled nursing setting.

You will coordinate interdisciplinary assessments, develop care plans, and maintain documentation while supporting CMS/PDPM requirements and reimbursement processes. Strong organization and communication are essential for timely, compliant submissions.

Qualifications

• Current RN or LPN license is required by state regulations.
• Experience coordinating MDS in skilled nursing or long‑term care preferred.
• Knowledge of CMS regulations, RAI guidelines, and MDS requirements.

Responsibilities

• Complete and submit MDS assessments accurately and within regulatory timeframes.
• Coordinate resident assessment with nursing, therapy, dietary, social services and others.
• Develop, review and update individualized resident care plans.
• Monitor assessment schedules and CMS/state requirements.
• Maintain accurate clinical documentation and assessment records.
• Review resident medical records for supporting documentation.
• Participate in interdisciplinary care plan meetings and assessments.
• Identify changes in resident conditions affecting outcomes and care planning.
• Collaborate with billing/reimbursement for accurate payment classification.
• Monitor quality measures and assist with quality improvement initiatives.
• Educate staff on MDS documentation and compliance.
• Maintain HIPAA confidentiality.
• Perform other duties as assigned.
