Job summary

Qualifications

• Active RN or LPN license as required by state regulations.
• Experience coordinating MDS in a skilled nursing or long-term care setting preferred.
• Knowledge of CMS, RAI, and MDS assessment requirements; strong documentation abilities.

Responsibilities

• Complete and submit MDS assessments accurately and within regulatory timeframes.
• Coordinate the resident assessment process with nursing, therapy, dietary, social services, and other departments.
• Develop, review, and update individualized resident care plans based on assessments.
• Monitor assessment schedules and ensure CMS/state compliance.
• Maintain clinical documentation and assessment records; review records for supporting documentation.
• Participate in interdisciplinary care plan meetings and resident assessments.
• Collaborate with billing/reimbursement teams to support accurate classification and reimbursement.
• Educate staff on MDS documentation, assessment requirements, and compliance standards.
