Job summary

Evergreen Healthcare Group in Seattle is seeking a MDS Coordinator Nurse (RN) to lead the development of the MDS, CAAs and care plans in accordance with state and federal requirements. You will participate as part of the Nursing Center Interdisciplinary Team (IDT) for Medicare management.

Qualifications

• Must have current RN or LPN licensure in the state of employment.
• Computer aptitude with EMR systems for this position.
• Prefer 2+ years in long-term care and RAI/MDS knowledge.

Responsibilities

• Coordinate the IDT for timely completion of assessments.
• Use MDS Scheduler to plan ARDs and communicate selections.
• Complete CAAs and care plans aligned with current MDS.
• Ensure documentation supports MDS coding and Medicare utilization.

Job Summary:

Seattle Medical Post Acute Care is seeking a MDS Coordinator Nurse (RN) to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. Participates as part of the Nursing Center Interdisciplinary Team (IDT) for Medicare management as assigned.

Essential Functions:

• Coordinate the IDT in timely completion of the assessments.
• Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
• Communicate ARD selection for specific MDSs to IDT.
• Complete Care Area Assessments (CAAs) as required.
• Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
• Monitor the overall documentation in the medical record to validate that it supports MDS coding.
• Participate in Medicare utilization management as assigned.

Education, Licensure, and Experience:

• Required: Current LPN or RN Licensure in the State of Employment
• Required: Computer aptitude with the EMR for this position
• Preferred: Two years’ experience working in a long-term care
• Preferred: Experience with Residence Assessment Instrument (RAI) process and MDS certification
• Preferred: Point Click Care (PCC) experience
