A leading healthcare provider in Longview is seeking an experienced MDS Coordinator Nurse to manage the development and completion of resident assessments. The role includes coordinating interdisciplinary team assessments and participating in Medicare management. Ideal candidates must have current LPN or RN Licensure and experience in long-term care. Qualifications

• Current LPN or RN Licensure in the State of Employment is required.
• Computer aptitude with the EMR for this position is required.
• Two years’ experience working in long-term care is preferred.
• Experience with Residence Assessment Instrument (RAI) process is preferred.
• Point Click Care (PCC) experience is preferred.

Responsibilities

• Coordinate the IDT in timely completion of the assessments.
• Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
• Complete Care Area Assessments (CAAs) as required.
• Complete CAAs as required.
• Complete care plan items relevant to current MDS and attend meetings.
• Monitor documentation to validate MDS coding.
• Participate in Medicare utilization management as assigned.

Job Summary

Wheatcrest Hills Healthcare Center is seeking a MDS Coordinator Nurse to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. Participates as part of the Nursing Center Interdisciplinary Team (IDT) for Medicare management as assigned.

Essential Functions

• Coordinate the IDT in timely completion of the assessments.
• Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
• Communicate ARD selection for specific MDSs to IDT.
• Complete Care Area Assessments (CAAs) as required.
• Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
• Monitor the overall documentation in the medical record to validate that it supports MDS coding.
• Participate in Medicare utilization management as assigned.

Education, Licensure, and Experience

• Required: Current LPN or RN Licensure in the State of Employment
• Required: Computer aptitude with the EMR for this position
• Preferred: Two years’ experience working in a long‑term care
• Preferred: Experience with Residence Assessment Instrument (RAI) process and MDS certification
• Preferred: Point Click Care (PCC) experience
