Job summary

Evergreen Healthcare Group is seeking an LPN MDS Coordinator at Pahrump Health and Rehabilitation Center to lead and coordinate MDS, CAAs, and care plans in line with state and federal requirements. You will work with the Nursing Center IDT for Medicare management.

Responsibilities include coordinating assessments, planning ARDs, and ensuring accurate documentation to support MDS coding and utilization management.

Qualifications

• Current LPN licensure in the state of employment is required.
• Computer aptitude with the EMR for this position is required.
• Two years’ experience in a long-term care setting is preferred.
• Experience with the Residence Assessment Instrument (RAI) process and MDS certification is preferred.
• Experience with PCC is preferred.

Responsibilities

• Coordinate the IDT in the timely completion of assessments.
• Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
• Communicate ARD selection for specific MDSs to IDT.
• Complete Care Area Assessments (CAAs) as required.
• Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
• Monitor documentation in the medical record to validate MDS coding and participate in Medicare utilization management.

Job Summary

Pahrump Health and Rehabilitation Center is seeking an LPN MDS Coordinator to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. Participates as part of the Nursing Center Interdisciplinary Team (IDT) for Medicare management as assigned.

Essential Functions

• Coordinate the IDT in timely completion of the assessments.
• Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
• Communicate ARD selection for specific MDSs to IDT.
• Complete Care Area Assessments (CAAs) as required.
• Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
• Monitor the overall documentation in the medical record to validate that it supports MDS coding.
• Participate in Medicare utilization management as assigned.

Education, Licensure, and Experience

• Required: Current LPN Licensure in the State of Employment
• Required: Computer aptitude with the EMR for this position
• Preferred: Two years’ experience working in a long-term care
• Preferred: Experience with Residence Assessment Instrument (RAI) process and MDS certification
• Preferred: Point Click Care (PCC) experience
