Job summary

Laramie Health and Rehabilitation is seeking a MDS Coordinator Nurse to lead the development and completion of the MDS, CAAs, and Care Plans in accordance with state and federal requirements, while participating in the Nursing Center IDT.

You will coordinate IDT tasks, plan ARDs, ensure documentation supports MDS coding, and participate in Medicare utilization management to optimize patient care and compliance.

Qualifications

• Current RN or LPN licensure in the state of employment.
• Proficient with EMR systems for this position.
• Two years’ long-term care experience preferred.
• Experience with RAI/MDS process and PCC is a plus.

Responsibilities

• Coordinate the IDT to ensure timely completion of MDS, CAAs, and care plans.
• Use MDS Scheduler to plan and set ARDs (Assessment Reference Dates).
• Communicate ARD selections for specific MDSs to the IDT.
• Complete CAAs and care plan items relevant to the current MDS.
• Monitor documentation to validate MDS coding accuracy.
• Participate in Medicare utilization management as assigned.

Job Summary

Laramie Health and Rehabilitation is seeking a MDS Coordinator Nurse to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. Participates as part of the Nursing Center Interdisciplinary Team (IDT) for Medicare management as assigned.

Essential Functions

• Coordinate the IDT in timely completion of the assessments.
• Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
• Communicate ARD selection for specific MDSs to IDT.
• Complete Care Area Assessments (CAAs) as required.
• Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
• Monitor the overall documentation in the medical record to validate that it supports MDS coding.
• Participate in Medicare utilization management as assigned.

Education, Licensure, and Experience

• Required: Current LPN or RN Licensure in the State of Employment
• Required: Computer aptitude with the EMR for this position
• Preferred: Two years’ experience working in a long-term care
• Preferred: Experience with Resident Assessment Instrument (RAI) process and MDS certification
• Preferred: Point Click Care (PCC) experience
