Job summary

Evergreen Healthcare Group is seeking an LPN MDS Coordinator for Pahrump Health and Rehabilitation Center to manage and coordinate the Resident Assessment Instrument (RAI), the Minimum Data Set (MDS), CAAs and Care Plans in line with state and federal requirements. You will participate with the Nursing Center Interdisciplinary Team for Medicare management as assigned.

The role supports the IDT, coordinating ARDs and ensuring documentation accuracy to reflect MDS coding.

Qualifications

• Current LPN licensure in the state of employment.
• Computer aptitude with the EMR for this position.
• Two years’ experience working in a long-term care (preferred).
• Experience with Residence Assessment Instrument (RAI) process and MDS certification (preferred).
• PCC experience (preferred).

Responsibilities

• Coordinate the IDT in timely completion of the assessments.
• Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
• Communicate ARD selection for specific MDSs to IDT.
• Complete Care Area Assessments (CAAs) as required.
• Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
• Monitor the overall documentation in the medical record to validate that it supports MDS coding.
• Participate in Medicare utilization management as assigned.
