Job summary

Laramie Health and Rehabilitation is seeking a MDS Coordinator Nurse to oversee the development and completion of the Resident Assessment Instrument (MDS), CAAs, and care plans in line with state and federal requirements. You will join the Nursing Center IDT for Medicare management as assigned.

Essential duties include coordinating the IDT, planning ARDs, and ensuring accurate MDS documentation; preferred experience with RAI/MDS and PCC.

Qualifications

• Current LPN or RN licensure in the state of employment.
• Proficient with EMR systems.
• Two years experience in long-term care (preferred).
• Experience with RAI/MDS process and certification (preferred).
• Point Click Care experience (preferred).

Responsibilities

• Coordinate the IDT to ensure timely completion of assessments.
• Use MDS Scheduler to plan ARDs.
• Communicate ARD selections to IDT.
• Complete CAAs as required.
• Complete care plan items and attend care plan meetings.
• Monitor documentation to support MDS coding.
• Participate in Medicare utilization management.

Job Summary

Laramie Health and Rehabilitation is seeking a MDS Coordinator Nurse to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. Participates as part of the Nursing Center Interdisciplinary Team (IDT) for Medicare management as assigned.

Essential Functions

• Coordinate the IDT in timely completion of the assessments.
• Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
• Communicate ARD selection for specific MDSs to IDT.
• Complete Care Area Assessments (CAAs) as required.
• Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
• Monitor the overall documentation in the medical record to validate that it supports MDS coding.
• Participate in Medicare utilization management as assigned.

Education, Licensure, and Experience

• Required: Current LPN or RN Licensure in the State of Employment
• Required: Computer aptitude with the EMR for this position
• Preferred: Two years’ experience working in a long-term care
• Preferred: Experience with Resident Assessment Instrument (RAI) process and MDS certification
• Preferred: Point Click Care (PCC) experience
