Job summary

Citrus Health Group is seeking a LVN/RN MDS Nurse to conduct and collect data for resident assessments in compliance with federal, state, and local regulations. The role collaborates with nursing staff, physicians, and the MDS Coordinator to implement RAI processes and care plans.

The position requires current CA licensure, nursing degree, and experience in resident assessment, with strong leadership and teaching abilities, infection prevention experience, and solid communication skills.

Qualifications

• Must hold active California RN or LVN license.
• Completed nursing program and degree from accredited college/university.
• Preferably 1+ year in resident assessment position.
• Demonstrated leadership and teaching abilities in nursing.
• Strong planning, organizing, and directing skills.
• Infection prevention experience required.
• Excellent written and oral communication.
• Knowledge of elder/chronically ill care needs.
• Ability to teach adult learners and use basic computer skills.
• Experience coordinating infection control activities.

Responsibilities

• Conduct and collect information to assist in development of the MDS and implementation of care plans.
• Participate in facility surveys by authorized government agencies.
• Work with IDT to develop comprehensive resident assessment and care plan.
• Evaluate resident condition and medical data to determine assessment needs.
• Coordinate scheduling of outside participants in the care plan team.
• Plan and conduct in-service training on assessment skills and techniques.
• Ensure completeness and accuracy of assessments and awareness of penalties for false certification.
• Assist in developing written care plans with measurable objectives and timetables.
• Ensure timely MDS assessments and care plan reviews.
• Ensure initial assessment is completed within the RAI timeframe.
• Ensure care plan is completed within 21 days of admission.
• Encourage resident and family involvement in care planning.
• Attend meetings and participate in training sessions as scheduled.
