Vantage Care Management is seeking an MDS Coordinator for the West Springfield, MA location. The role coordinates and completes accurate resident assessments in compliance with state and federal regulations, partnering with the interdisciplinary team to support quality care outcomes and reimbursement accuracy.

This position offers competitive pay plus comprehensive benefits, including medical, dental, vision, 401(k), PTO and sick time, with a pay range of $65,000.00 – $90,000.00 per year.

Qualifications

• Active Massachusetts RN or LPN license
• MDS experience preferred
• Strong organizational and analytical skills
• Knowledge of long-term care regulations

Responsibilities

• Coordinate and complete MDS assessments and care plans
• Ensure accuracy, timeliness, and regulatory compliance
• Collaborate with nursing, therapy, and interdisciplinary teams
• Monitor documentation to support reimbursement and quality metrics
• Participate in audits and quality improvement initiatives
