Job summary

Columbus Oaks Healthcare Community is seeking an experienced MDS Coordinator to join our long-term care unit. The role focuses on timely completion of the RAI process and care management from admission to discharge, in line with policy and federal/state guidelines.

The coordinator may, with guidance, help coordinate information systems and education for the clinical department. We value dedicated caregivers and offer a comprehensive benefits package.

Qualifications

• Current, unencumbered license to practice as an LPN/LVN or RN in this state.
• Three (3) years’ MDS experience in a nursing care facility preferred.

Responsibilities

• Responsible for timely and accurate completion of the RAI process and care management from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines.
• With direction from the VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.
