Job summary

Southern HealthCare Management is seeking an LPN with MDS experience to join our team as an MDS Coordinator. You will develop, implement, and coordinate the Resident Assessment Instrument (RAI) process to keep the center in compliance with OBRA, MDS guidelines, and physician orders.

The role focuses on ensuring regulatory alignment across local, state, and federal standards, supporting care teams, and maintaining timely documentation.

Qualifications

• Active, unrestricted LPN licensure in Florida
• Knowledge of Medicare/Medicaid, OBRA, RAI, and MDS processes preferred or prior long-term care nursing experience

Responsibilities

Entry Qualifications

• Active, unrestricted Licensed Practical Nurse LPN licensure in the state of practice required
• Working knowledge of Medicare and Medicaid programs, LTC payer systems, the Omnibus Budget Reconciliation Act (OBRA), the RAI process, and the MDS process preferred OR prior nursing in a long-term care facility
