Tutera Senior Living and Health Care is hiring an MDS Coordinator | LPN in Metropolis, Illinois. This role involves overseeing the RAI process and managing the care of seniors from admission to discharge, ensuring compliance with all regulations.

Qualifications

• Minimum one year of experience in LTC/SNF/AL/MC setting highly preferred.
• Excellent knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.

MDS Coordinator | LPN

What Will You Do in This Role?

As an MDS Coordinator | LPN, you will be responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge, in accordance with company policy and procedures, Federal, State and Certification guidelines, and all relevant entities. You will manage Minimum Data Set, discharge, and admission tracking and may coordinate information systems operations and education for the clinical department under direction from the Director of Nursing and VP of Clinical Reimbursement.

• Prior MDS experience highly preferred
• Minimum of one year of experience in a LTC/SNF/AL/MC setting highly preferred
• Excellent knowledge of Case‑Mix, the Federal Medicare PPS process, and Medicaid reimbursement highly preferred
• Must possess a thorough understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set
• Knowledge of the care planning process required
• Professional image in both appearance and behavior
• Excellent written and oral communication skills
