Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator (RN or LPN) to lead the MDS process from admission to discharge in a collaborative SNF setting. You will ensure timely and accurate RAI completion and care management, coordinating with the interprofessional team.

Candidates should have 2+ years in a Skilled Nursing Facility, strong knowledge of MDS 3.0, Medicare PPS, OBRA regulations, and excellent attention to detail and communication.

Qualifications

  • Graduate of an approved RN or LPN program and licensed in the state of practice.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
  • In-depth knowledge of MDS 3.0, Medicare PPS, Medicaid reimbursement and OBRA regulations.
  • Experience with care planning and quality indicators.
  • Strong attention to detail and excellent communication skills.

Responsibilities

  • Collaborate with the Interdisciplinary Team to assess resident needs and create MDS assessments and care plans.
  • Ensure accurate and timely documentation of the MDS process, including coding and medical record reviews.
  • Assist in coordinating daily stand-up meetings and review care and assessment schedules.
  • Stay updated on federal, state, and certification guidelines to ensure compliance.
  • Submit resident data to state and federal agencies and ensure all MDS documentation is complete.

Are you an experienced RN or LPN with expertise in the Minimum Data Set (MDS) process? Join our team as an MDS Coordinator in a dynamic, collaborative environment where you’ll play a key role in ensuring timely and accurate completion of the RAI process and care management from admission to discharge.

What You’ll Do
  • Collaborate with the Interdisciplinary Team to assess resident needs and create MDS assessments and care plans in accordance with state and federal guidelines.
  • Ensure accurate and timely documentation of the MDS process, including coding, medical record reviews, and interdisciplinary updates.
  • Assist in coordinating daily stand‑up meetings and review care and assessment schedules.
  • Stay updated on federal, state, and certification guidelines to ensure compliance with all regulations.
  • Submit resident data to state and federal agencies, and ensure all MDS documentation is accurate and complete.
What We’re Looking For
  • Graduate of an approved Registered Nurse (RN) or Licensed Practical Nursing (LPN) program and licensed in the state of practice.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
  • In‑depth knowledge of the MDS 3.0, Medicare PPS process, Medicaid reimbursement, and OBRA regulations.
  • Experience with care planning and quality indicators.
  • Strong attention to detail and excellent communication skills.
