Job summary

• Must hold current LPN or RN licensure in Florida.
• Minimum of 1 year of experience in long-term care, gerontology, or related field.

Responsibilities

• Coordinate and conduct thorough assessments of residents' medical, social, and psychological needs to develop and implement comprehensive care plans in collaboration with the healthcare team.
• Review and update Minimum Data Set (MDS) assessments, care plans, and resident records to ensure accuracy and compliance with regulatory requirements.
• Collaborate with caregivers, physicians, and other healthcare professionals to provide high-quality care and services that meet the unique needs of each resident.
• Identify and report any changes in residents' conditions, concerns, or complaints to the healthcare team and family members.
• Develop and maintain effective communication systems to ensure seamless coordination of care and services.
