Job summary

Mountain Ridge Rehabilitation and Healthcare Center in Black Mountain, NC, seeks an experienced MDS Coordinator to manage the written plan of care for each resident, ensuring accurate needs assessment and goals are met. You will collaborate with the MDS team to develop care plans, monitor status changes, and guarantee compliant documentation.

Qualifications

• Current State License as a Nurse.
• CPR Certification.
• One year experience as a Nurse in a long-term care setting (preferred).

Responsibilities

• Coordinate, manage, and monitor the written plan of care for each resident.
• Assist with development and review of comprehensive care plans.
• Ensure documentation meets regulatory and auditor requirements.
• Assist nursing management with coordination, management, and review of nurse's notes.
• Monitor resident status changes to ensure timely nursing or clinical involvement.
• Perform regular audits of documentation for accuracy.
• Complete necessary forms and reports.
• Maintain strict confidentiality of health information.
• Report hazardous conditions, damaged equipment, and supply issues to appropriate persons.
