Mountain Ridge Rehabilitation and Healthcare Center in Black Mountain, NC is seeking a dedicated MDS Coordinator to manage the written plan of care for each resident and ensure regulatory compliance. You will coordinate with the MDS team to develop comprehensive care plans, monitor status changes, and maintain accurate documentation.

The ideal candidate holds an RN license, CPR certification, and 1 year of long-term care nursing experience.

Qualifications

• Current State License as a Nurse.
• CPR certification is required.
• One year of experience as a nurse in a long-term care setting is preferred.

Responsibilities

• Keep abreast of current federal and state regulations and professional standards.
• Assist with development of comprehensive care plans in coordination with the MDS team.
• Coordinate, manage, and monitor the written plan of care for each resident.
• Assist nursing management with coordination, management, and review of nurse's notes.
• Monitor resident status changes to ensure timely nursing or clinical involvement.
• Ensure MDS and support documentation meet regulatory and auditor requirements.
• Perform regular audits of documentation for accuracy.
• Assist with the discharge process.
• Complete necessary forms and reports.
• Maintain confidentiality of sensitive health information.
• Report hazardous conditions, damaged equipment, and supply issues.

Job Summary

As an MDS Coordinator, you will be responsible for coordinating, managing, and monitoring the written plan of care for each resident, ensuring that their needs are accurately addressed and goals are achieved. You will work closely with the MDS team to develop comprehensive care plans, monitor resident status changes, and ensure accurate documentation. Your exceptional organizational skills and attention to detail will be essential in ensuring compliance with federal and state regulations, as well as professional standards.

Key Responsibilities

• Assist with the development of comprehensive care plans for residents in coordination with the MDS
• Coordinate, manage, and monitor the written plan of care for each resident

Requirements
