Qualifications

• Current State License as a Nurse.
• CPR Certification.
• One year experience as a Nurse in a long-term care setting (preferred).

Responsibilities

• Coordinate, manage, and monitor the written plan of care for each resident.
• Assist with development and review of comprehensive care plans.
• Ensure documentation meets regulatory and auditor requirements.
• Assist nursing management with coordination, management, and review of nurse's notes.
• Monitor resident status changes to ensure timely nursing or clinical involvement.
• Perform regular audits of documentation for accuracy.
• Complete necessary forms and reports.
• Maintain strict confidentiality of health information.
• Report hazardous conditions, damaged equipment, and supply issues to appropriate persons.

Mountain Ridge Rehabilitation and Healthcare Center, a Long-Term Care facility in Black Mountain, NC, is seeking a dedicated and experienced MDS Coordinator to join our team.

Job Summary

As an MDS Coordinator, you will be responsible for coordinating, managing, and monitoring the written plan of care for each resident, ensuring that their needs are accurately addressed and goals are achieved. You will work closely with the MDS team to develop comprehensive care plans, monitor resident status changes, and ensure accurate documentation. Your exceptional organizational skills and attention to detail will be essential in ensuring compliance with federal and state regulations, as well as professional standards.

Key Responsibilities

• Keep abreast of current federal and state regulations, as well as professional standards
• Assist with the development of comprehensive care plans for residents in coordination with the MDS
• Coordinate, manage, and monitor the written plan of care for each resident
• Assist nursing management with the coordination, management, and review of nurse's notes
• Monitor resident status changes to ensure timely nursing or clinical involvement
• Assure MDS and support documentation are accurate and meet regulatory and auditor requirements
• Perform regular audits of documentation to ensure accuracy
• Assist with the discharge process
• Complete necessary forms and reports
• Maintain strict confidentiality regarding sensitive health information
• Report hazardous conditions, damaged equipment, and supply issues to appropriate persons

Requirements

• Current State License as a Nurse
• C.P.R. Certified
• One year experience as a Nurse in a long-term care setting (preferred)
