Fletcher Rehabilitation and Healthcare Center in Fletcher, NC seeks a dedicated MDS Coordinator to join our long term care team. You will collaborate with the MDS team to develop care plans, monitor progress, and ensure accuracy of documentation and regulatory compliance.

This role requires RN licensure and CPR certification, with a focus on resident dignity and high-quality care. Experience in long-term care and familiarity with MDS processes is preferred.

Formación

• Requires current State RN license.
• CPR certification required.
• Knowledge of MDS process and regulatory standards preferred.

Responsabilidades

• Develop and coordinate comprehensive care plans with the MDS team.
• Monitor written plan of care and ensure goals are met for each resident.
• Review nurse notes to ensure care plan adherence and accuracy.
• Perform regular documentation audits for regulatory compliance.
• Assist with discharge processes and administrative reporting.
• Maintain confidentiality and follow infection control practices.
• Communicate effectively with residents, families, and staff.
