Job summary

Fletcher Rehabilitation and Healthcare Center in Fletcher, NC is hiring a full-time MDS Coordinator to ensure accurate resident assessments and care plans in compliance with regulations. You will coordinate with the nursing staff, monitor resident status, perform audits of documentation, oversee discharge processes, and participate in meetings to promote quality care for residents and families.

Experience with MDS processes and a current nursing license are required; CPR certification is also

Qualifications

• Current State Nurse license required.
• CPR certification required.
• Knowledge of MDS processes and related regulations.

Responsibilities

• Ensure compliance with federal and state regulations and professional standards.
• Coordinate, manage and monitor the written plan of care for each resident with the MDS.
• Monitor resident status changes and involve clinical team promptly.
• Maintain accurate documentation and perform regular audits.
• Assist with discharge processes and administrative reports.
• Maintain infection control practices and patient confidentiality.
• Communicate effectively with residents, families, peers and supervisors.
• Attend departmental meetings and trainings as directed.
