Job summary

Wisconsin Rapids Health Services is seeking a compassionate MDS Coordinator to lead RAI assessments and coordinate care plans, ensuring Medicare/Medicaid OBRA compliance. This is a working manager role with time on the floor with residents and staff.

Open to LPN candidates as well; RN wages up to $53.50/hr. This hybrid position offers growth opportunities and a team dedicated to resident-centered care.

Qualifications

• RN in good standing with state licensing board.
• Experience with PPS assessment and OBRA compliance.
• Excellent clinical assessment skills; strong organization and independent decision making.
• Strong interpersonal skills and proficiency with Microsoft products.

Responsibilities

• Direct and administer the RAI process to ensure accurate, timely resident assessments.
• Ensure assessments reflect residents’ physical, mental and psychosocial status and support optimal reimbursement.
• Be a working manager with time on the floor with residents and staff.
