Job summary

North Shore Health community, Wisconsin Rapids Health Services, is seeking a compassionate MDS Coordinator to lead the RAI process for accurate resident assessments. This working manager role requires time on the floor with residents and staff to ensure compliance and quality care.

RN licensure, geriatrics experience, PPS/OBRA knowledge, and strong assessment skills are essential. Hybrid work arrangement complements our mission of resident-centered excellence.

Qualifications

• Required RN license in good standing; clinical geriatrics experience desired.
• Demonstrated PPS assessment and OBRA compliance knowledge.
• Excellent clinical assessment skills and strong organizational abilities.
• Ability to lead the RAI process with on-floor collaboration and reporting.

Responsibilities

• Direct and administer the Resident Assessment Instrument (RAI) process to ensure accurate, timely resident assessments per payor requirements.
• Ensure documentation supports services and optimal reimbursement; monitor assessment accuracy.
• Work as a working manager with time on the floor alongside residents and staff.
