Job summary

YAD Management is seeking an MDS Coordinator for a full-time position at our Asheboro, NC facility. The role involves coordinating and monitoring the written plan of care for residents, ensuring MDS documentation meets regulatory standards, and supporting discharge and administrative processes.

Ideal candidates will hold a current State Nurse License and CPR certification, with preferred experience in a long-term care environment.

Qualifications

• Current State Nurse License (active) required.
• CPR certification required or in process.

Responsibilities

• Coordinate and monitor the plan of care for each resident.
• Ensure MDS documentation meets regulatory and auditor requirements.
• Assist with development of comprehensive care plans in coordination with MDS.
• Review nurse notes to ensure care plans are followed.
• Conduct regular audits of documentation for accuracy.
• Support discharge processes and administrative reporting.
• Maintain infection control and safety practices.
• Protect resident confidentiality and privacy.
• Communicate effectively with residents, families, peers, and supervisors.
• Participate in departmental meetings and trainings.
