Job summary

Asheboro Rehabilitation and Healthcare Center in Asheboro, NC is seeking an MDS Coordinator for full-time on-site work. You will develop care plans with the MDS team, monitor the written plan of care for each resident, and ensure accurate regulatory documentation while coordinating with nursing management.

Qualifications

• Current State License as a Nurse.
• CPR certification required.
• One year experience as a Nurse in a long-term care setting is preferred.

Responsibilities

• Keep abreast of current federal and state regulations and professional standards.
• Develop comprehensive care plans with the MDS to address residents' needs.
• Coordinate, manage and monitor the written plan of care for each resident.
• Review nurse notes to ensure care plan adherence and timely involvement.
• Monitor resident status changes and ensure appropriate clinical involvement.
• Ensure MDS documentation accurately reflects residents and meets regulatory needs.
• Perform regular audits of documentation for accuracy.
• Assist nursing management with discharge processes.
• Complete necessary forms and reports; maintain infection control practices.
• Maintain confidentiality and respond to call lights promptly.
• Interact professionally with residents, families, and staff; attend meetings.
