Responsibilities

• Stay updated on federal and state regulations and professional standards.
• Assist with development of comprehensive care plans for residents in coordination with the MDS.
• Coordinate, manage and monitor the written plan of care for each resident.
• Assist nursing management with nursing notes to determine care plan adherence.
• Monitor resident status changes to ensure timely nursing/clinical involvement.
• Assist with discharge processes and required administrative tasks.
