Job summary

Manchester Center for Rehabilitation and Healing seeks an experienced MDS Coordinator to ensure accurate MDS data, CAAs, and care plans for residents in a skilled nursing setting.

The role requires an active RN license and at least two years of clinical experience, including long-term care. You will work with the IDT and families to optimize discharge planning and regulatory compliance.

Qualifications

• Active RN license in state of practice.
• Minimum two years of clinical experience in a health care setting.
• Minimum one year of experience in a long-term care setting.
• Prior experience as an MDS coordinator is preferred.

Responsibilities

• Oversee completion of the MDS, CAAs and Care Plans in accordance with current regulations.
• Act as in-house Case Manager coordinating care pathways and timely communication with residents, families and payers.
• Communicate regulatory risk and facility performance to Administrator and Director of Nursing.
• Collaborate with therapy staff to determine ARD and capture the RUG score.
• Ensure understanding of MDS requirements for Medicare, Managed Care and Medicaid.
• Ensure timely electronic submission of MDS to state databases and review validation reports.
• Lead Care Management process with residents, IDT and family to resolve discharge barriers.
• Educate IDT members on the RAI process and train on updated MDS procedures.
• Analyze QI/QM data with DON to identify trends and improve outcomes.
• Participate in facility committees as required.
