Winchester Rehabilitation and Nursing Center in Winchester, MA is seeking an experienced MDS Coordinator to oversee resident assessments and CAAs, coordinating with the interdisciplinary team to develop comprehensive care plans. This full-time role includes supporting survey readiness and ensuring compliance with regulations.

The successful candidate will hold a MA RN or LPN license with a minimum of two years in MDS/PPS and be committed to resident-centered care in a 120-bed facility.

Qualifications

• Current, unencumbered Massachusetts RN or LPN license.
• Minimum of two years of MDS and PPS experience.
• Strong organizational and communication abilities.
• Ability to coordinate with an interdisciplinary healthcare team.

Responsibilities

• Conduct and coordinate development of resident assessments.
• Perform and coordinate resident assessments per regulations.
• Implement Care Area Assessments (CAAs).
• Collaborate with the care team to develop individualized care plans.
• Support facility survey readiness and process.
