The position is full-time, daytime hours in a Kansas facility, with collaboration across nursing, therapy, social services, and dietary teams to optimize resident care.

Qualifications

• RN or LPN license required (state license)
• Experience as an RN in healthcare setting; MDS coordination preferred
• Strong clinical assessment knowledge and nursing principles for long-term care
• Proficiency with MDS software and electronic health records
• Excellent organizational skills and ability to meet deadlines
• Clear communication and interpersonal skills for team collaboration

Responsibilities

• Coordinate MDS assessments for all residents in compliance with federal/state regulations and facility policies
• Conduct resident pre-assessment and assessment using standardized tools across physical, psychological, and psychosocial domains
• Collaborate with RNs, therapists, social workers and dietary staff to develop individualized care plans
• Ensure accuracy and completeness of MDS documentation and regulatory compliance
• Schedule MDS assessments and ensure timely completion for admissions, quarterly reviews, significant changes and discharges
• Enter data in MDS software, generate reports, and maintain resident records
• Monitor resident condition changes and update MDS as needed for accurate reimbursement
• Participate in care conferences to discuss plans, progress and changes in condition
• Coordinate with billing to ensure correct Medicare/Medicaid billing based on MDS data
• Provide education and training to staff on MDS protocols and regulatory changes
• Complete reports on time and perform other duties as assigned
