Job summary

Fort Lauderdale Health and Rehabilitation Center is seeking a Full-time MDS Coordinator to lead the resident assessment process in accordance with state regulations and facility goals. You will coordinate the RAI process, maintain policies, and develop a quality assurance program while ensuring timely completion and reporting.

Strong knowledge of OBRA, Medicare/Medicaid requirements, and QI reporting is essential.

Qualifications

• Must have 2+ years of MDS Coordinator experience in a skilled nursing facility.
• RN license active in this state is required.
• Current COVID vaccination, CPR, licensing, and resume/references are required.
• Level II background check, drug screening and reference check on hire.

Responsibilities

• Coordinate the development and completion of the RAI process per state and facility policies.
• Maintain and update policies governing the RAI process.
• Develop and maintain an ongoing quality assurance program for resident assessments.
• Monitor QI/QM reports and implement corrective actions as needed.
• Ensure current RAI Manual is available to those completing portions of the MDS.
• Ensure timely completion and transmission of assessments.
• Chair Daily Case Management Meeting.
• Chair Weekly UR Meeting.
• Participate in discharge planning as necessary.
