Job summary

The MDS Coordinator is responsible for overseeing the completion and accuracy of the Minimum Data Set (MDS) assessments in compliance with federal and state regulations.

You will conduct MDS assessments, coordinate with interdisciplinary teams to develop care plans, train staff on MDS processes, monitor quality indicators, and keep abreast of regulatory changes to ensure ongoing compliance.

This role requires attention to detail and strong communication to ensure excellent resident care.

Qualifications

• RN license or eligible for licensure in the relevant state
• Experience in long-term care and MDS coordination preferred
• Strong communication and organizational skills

Responsibilities

• Conduct MDS assessments and ensure timely submissions
• Collaborate with interdisciplinary teams to develop care plans
• Provide training and support to staff on MDS processes
• Monitor quality indicators and implement improvement strategies
• Maintain up-to-date knowledge of regulatory changes
