The role leads the RAI process, collaborates with interdisciplinary team, coordinates care planning, and ensures timely Medicare/Medicaid documentation while supporting a patient-centered care environment.

Qualifications

• RN license in the state of employment is required.
• Experience with MDS/RAI and regulatory guidance is preferred.
• Strong leadership and interdepartmental collaboration skills.
• Excellent communication and organizational abilities.

Responsibilities

• Coordinates the RAI process and MDS assessments.
• Ensures accurate, timely MDS documentation per regulations.
• Collaborates with the interdisciplinary team for care planning.
• Maintains schedules and conducts MDS-related education sessions.
• Manages communications with insurance case managers and families.
