Job summary

Brookwood Skilled Nursing & Therapy is seeking a dedicated Case Manager to lead, coordinate, and support our nursing team in Oklahoma City. The role focuses on MDS planning, pre-admission screenings, discharge coordination, and ensuring residents receive the highest quality care.

The position requires an Oklahoma RN or LPN license with CPR/First Aid. Prior SNF/LTC experience is strongly preferred, and you will work in a hands-on, collaborative environment with opportunities for career

Qualifications

• Current and unencumbered Oklahoma RN or LPN license.
• Up to date First Aid and CPR certification.
• One+ year of experience in a Skilled Nursing Facility or Long Term Care environment strongly preferred.

Responsibilities

• Lead daily interdisciplinary meetings to monitor therapy minutes, RUG changes, and resident status.
• Perform pre-admission screenings to determine level of care and eligibility for Medicare-subsidized services.
• Plan and collect MDS assessments data per Medicare schedule.
• Coordinate discharge planning.
• Conduct daily rounds to evaluate resident care and advise nursing staff.
