Job summary

Topaz Fiscal Services LLC in Atlanta, GA seeks an experienced MDS Coordinator to manage the RAI process, complete accurate MDS/CAAs, and lead care planning in a long-term care setting.

You will coordinate ARD planning with interdisciplinary teams, host Casemix and Utilization Review meetings, and ensure compliance with federal/state regulations while protecting resident dignity.

Strong attention to detail, leadership, and communication are essential to succeed in this role.

Qualifications

• Must possess a current, unencumbered, active nursing license in the state.
• Registered Nurse license preferred.
• 2 years’ experience in long-term care preferred.
• 6 months experience in rehabilitative/restorative nursing preferred.
• 2 years’ experience in RAI process completion preferred.
• Ability to make independent decisions when warranted.
• Demonstrates leadership and organizational skills.
• Familiarity with Point Click Care and basic computer literacy.

Responsibilities

• Maintain current MDS status per laws and facility policies.
• Schedule assessments and coordinate communication with departments and families.
• Monitor resident condition changes and coordinate MDS reassessment.
• Strategically set ARD with interdisciplinary team to reflect resident needs.
• Complete MDS and CAAs accurately per guidelines.
• Attend daily Casemix Meetings, weekly Utilization Review Meetings, monthly Triple Check Meetings.
• Maintain assessments for at least 15 months per federal regulations.
• Transmit MDS data electronically to the state per regulations.
• Adhere to facility policies and protect resident dignity and privacy.
• Participate in surveys and contribute to care planning.
