Apploi is seeking an MDS Coordinator to ensure accurate Medicare/Medicaid case-mix documentation and timely care plan development. You will oversee MDS/ PPS submission, coordinate interdisciplinary reviews, and integrate nursing, dietary, social, and rehabilitation services to optimize reimbursement.

The role requires licensure as RN or LPN, with long-term care experience and restorative certification within six months if not already held.

Formación

• Must have current unencumbered state licensure as RN or LPN.
• Experience in long-term care settings required.
• Restorative Certification should be obtained within 6 months of hire if not already certified.

Responsabilidades

• Perform MDS assessments and documentations for PPS/PDPM and RUG categorization.
• Develop and update care plans on admission, quarterly, and as needed within regulatory timeframes.
• Schedule and conduct interdisciplinary care conferences; ensure MDS reviews precede conferences.
• Coordinate restorative nursing program and ensure alignment with care plans.
