Job summary

The MDS Coordinator ensures accurate and timely Medicare/Medicaid case-mix and care plan documentation to support appropriate reimbursement. The role integrates nursing, dietary, social recreation, restorative and physician services to optimize PDPM and RUG categorization.

The position requires leadership in MDS assessments, care planning, and interdisciplinary coordination, with responsibility for documenting PPS, quarterly and annual changes, and restorative nursing program oversight.

Qualifications

• RN or LPN with current, unencumbered state license.
• Long-term care experience required.
• Restorative certification within six months if not already certified.
• Ability to read, write, speak and understand English.

Responsibilities

• Assess health status and determine level of care for new admissions.
• Ensure accurate and timely completion of all MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Maintain schedule of MDS assessments with proper reference dates.
• Complete care plans on admission, quarterly and as needed.
• Schedule and conduct resident care conferences and ensure MDS reviews before conferences.
• Oversee restorative nursing measures reflected on care plans.
