Job summary

Mountain Ridge Rehabilitation and Healthcare Center in Black Mountain, NC seeks an experienced MDS Coordinator to manage the written plan of care for each resident and ensure accurate, compliant documentation.

You will collaborate with the MDS team, develop comprehensive care plans, monitor status changes, and assist with discharge processes while maintaining confidentiality and regulatory standards.

Qualifications

• Must hold a current State Nurse license.
• CPR certification required.
• One year of long-term care nursing experience preferred.

Responsibilities

• Coordinate, manage, and monitor the written plan of care for each resident.
• Assist with development of comprehensive care plans with the MDS team.
• Ensure documentation complies with regulatory and auditor requirements.
• Perform regular audits of documentation for accuracy.
• Support discharge processes and report hazards and equipment issues.
