Mountain Ridge Rehabilitation and Healthcare Center in Black Mountain, NC is seeking an experienced MDS Coordinator to oversee the written plan of care for residents, ensure accurate documentation, and maintain regulatory compliance.

You will collaborate with the MDS team, monitor status changes, support discharge processes, and perform regular audits while protecting resident privacy and confidentiality.

Qualifications

• Current State License as a Registered Nurse (RN)
• CPR certification required
• Experience in long-term care preferred (1 year)

Responsibilities

• Stay abreast of federal and state regulations and professional standards
• Coordinate and develop comprehensive care plans with the MDS team
• Manage and monitor written plans of care for each resident
• Assist nursing management with coordination and review of nurse's notes
• Monitor resident status changes to ensure timely nursing or clinical involvement
• Ensure MDS and supporting documentation meet regulatory/audit requirements
• Perform regular audits of documentation for accuracy
• Assist with discharge processes
• Complete necessary forms and reports
• Maintain strict confidentiality of health information
• Report hazardous conditions, equipment damage, and supply issues

Job Summary:

As an MDS Coordinator, you will be responsible for coordinating, managing, and monitoring the written plan of care for each resident, ensuring that their needs are accurately addressed and goals are achieved. You will work closely with the MDS team to develop comprehensive care plans, monitor resident status changes, and ensure accurate documentation. Your exceptional organizational skills and attention to detail will be essential in ensuring compliance with federal and state regulations, as well as professional standards.

Key Responsibilities:

• Keep abreast of current federal and state regulations, as well as professional standards


• Assist with the development of comprehensive care plans for residents in coordination with the MDS


• Coordinate, manage, and monitor the written plan of care for each resident


• Assist nursing management with the coordination, management, and review of nurse's notes


• Monitor resident status changes to ensure timely nursing or clinical involvement


• Assure MDS and support documentation are accurate and meet regulatory and auditor requirements


• Perform regular audits of documentation to ensure accuracy


• Assist with the discharge process


• Complete necessary forms and reports


• Maintain strict confidentiality regarding sensitive health information


• Report hazardous conditions, damaged equipment, and supply issues to appropriate persons

Requirements:

• Current State License as a Nurse


• C.P.R. Certified


• One year experience as a Nurse in a long-term care setting (preferred)
