Journey is seeking a skilled MDS Coordinator in Georgia to support the resident assessment process by completing MDS sections and related CAAs. The role helps develop individualized care plans by identifying resident needs and expected outcomes in line with federal and state regulations.
The position requires RN licensure in Georgia, solid MDS 3.0 knowledge, and strong teamwork to coordinate care plans with an interdisciplinary team.
The MDS Coordinator supports the resident assessment process by completing the appropriate sections of the Minimum Data Set (MDS) and related Care Area Assessments (CAAs). This role also helps develop individualized care plans by identifying resident needs and expected outcomes in accordance with federal and state regulations.
