Job summary

St. Anne Home in Pennsylvania is seeking a Registered Nurse Assessment Coordinator – RNAC to manage the MDS process from admission to discharge, collaborating with the IDT to ensure accurate documentation and timely care planning.

This full-time on-site role requires PA RN licensure, 1 year MDS, LTC and Care Plan experience; compensation is $40–$43 per hour with benefits including health insurance, 403(b), and paid holidays.

Qualifications

• Licensed as a Registered Nurse in Pennsylvania.
• Minimum of 1 year of MDS experience.
• Minimum of 1 year of Long-Term Care experience.
• Minimum of 1 year of Care Plan experience.

Responsibilities

• Complete all required sections of the MDS.
• Lead Care Plan meetings and monitor documentation.
• Ensure medical records support MDS coding.
• Collaborate with the IDT to assess needs and plan care.
• Obtain and verify Skilled Certification/Re-Certification documentation.
